We present a case of a 45 years old female with extruded lumbar disc herniation that presented with acute right sciatica and L5 paresis. The patient refused surgical treatment although she was a good candidate. She underwent ozone therapy with ultrasound-guided caudal and foraminal infiltrations (20ml, 20 \u00b5g\/mL) plus paravertebral infiltrations (10 ml, 20 \u00b5g\/mL).
